Significance of Axillary Lymph Node Level Metastasis in Operable Breast Cancer

Abstract
The status, of axillary lymph nodes has been accepted to be an important indicator of the prognosis for patients with operable breast cancer. Sixiy mastectomies with complete axillary dissection were analyzed for significance of involvement at
axillary
levels and number of metastatic nodes with 5-year disease- free survival rates. No lymph node metastasis was found in 29 of the specimens while the remainder (31 cases) had at least one affected lymph node.
The average number of nodes identified was 20.1. The average number of nodes recovered per level was 12.5 at level I, 4.2 at level II, 2.4 at level III and 1.0 for interpectoral node. There was no patient who had positive nodes at levels II
and/or
III
without metastasis to level I(no skip metastasis). Most cases of tumor size over 5 cm or number of metastatic nodes over 9 showed tendency of metastasis to level III. There was no significant difference among groups according to metastatic
highest
level
but significant difference among groups according to metastatic highest level but significant differnce among groups according to number of metastatic nodes. (LN=O vs LN¡Ã4, LN 1-3 vs LN¡Ã4 p<0.05) These findings suggest that low axillary
dissection in
conservative surgery for breast cancer is sufficient for evaluation of higher level status. Also number of metastatic lymph node is still important indicator for prognosis of breast cancer.
